Simply Home Health LLC is the newest of the 3 Simply Companies. We currently operate in Illinois but soon in Colorado as well. This Joint Commission accredited home health agency uses, whenever possible, the same therapists who treated a patient in a skilled nursing facility as the clinician who sees the patient after being discharged to home health.

This Continuity of Care is a win-win for patients and family members because they keep the same relationship they had in the SNF, which eliminates the learning curve always associated with the hand off from SNF to home health. When a hospital discharges a patient to one of the 3 SNF’s in Illinois where we have implemented this concept (Aurora Rehab, Arlington rehab and Rolling Hills Manor) a patient can request, and get, the same therapist to follow them home.

The Otago Balance Retraining Falls Prevention Program

Simply Home Health is the first Illinois home health agency to implement the Otago Medical School Falls Prevention Program. This New Zealand tested program was endorsed by the CDC in 2008 as one of only six geriatric falls prevention programs with enough evidence to prove it was effective in reducing falls. Our community based patients are taught this program by our nurses and therapists while on caseload and they then receive post discharge follow up phone calls and visits (which are not billed to Medicare) to make sure the exercise program becomes part of a new and improved life style. Our version of Otago has been improved by using our own strength measurement test that avoids fatigue while still producing an accurate set of numbers that tell us increases and decreases in the participant’s strength and endurance.

Training and Technology

We invest heavily in providing ongoing training and mentoring to all our nurses and therapists to ensure that they are up to date on all the latest clinical techniques, especially falls prevention. Our agency is 100% electronic and we use the Kinnser on-line treatment and documentation system, which minimizes lost or late paperwork and enables on-line coordination of services.
